Attention all Middle School Students, who are interested in playing a sport!
LISTEN UP!
If you are interested in playing a sport for the upcoming 2023-2024 school year, please stop by the Athletics Director Office to pick up an Athletic packet. This packet includes the following, Sports Interest Form, Physical Form, Sports fees form(s), medical release form, bus waiver form, athletic photos form, Athletics Handbook, Athletics Code of Conduct Form, FCA information, Sports Uniform forms, as well as conditioning and practice sites information for each sport, pick up roster, and parent contact information.
All FORMS are due back 2 weeks before the sport(s) start. If you do not turn in the requested forms, you will not be permitted to play ANY sport.
